Treating Neck and Upper Back Pain from Poor Sleep Posture

John, a 33-year-old office worker, came to East Sussex Osteopaths complaining of persistent
neck stiffness and upper back pain that had been worsening over several months. The
discomfort was most noticeable in the mornings and often left him with limited range of
motion throughout the day.

After a thorough case history and physical assessment, it became clear that John’s symptoms
were largely related to poor sleep posture and the use of an unsupportive pillow. He reported
waking up with a tight neck, occasional tension headaches, and discomfort that radiated into
the tops of his shoulders.

Osteopathic Assessment and Treatment

John presented with increased muscular tension across the upper trapezius and cervical spine,
along with reduced mobility in the mid-thoracic region. His head posture was slightly
forward, a pattern often worsened by poor pillow support and long hours at a desk.
We began treatment with three osteopathic sessions spaced over four weeks. Each session
included:

 Soft tissue techniques to relieve tension in the neck and shoulders
 Gentle joint articulation to improve mobility in the cervical and thoracic spine
 Postural education to correct sleep habits, pillow positioning and trying pillow
thickness.

In addition to osteopathic care, John also received two therapeutic massage sessions from
our massage therapist Sarahjane focusing on the upper back, neck, and shoulder girdles.
These helped further reduce muscle tightness and promote circulation to the affected areas.

Outcome

By the end of his treatment plan, John reported that suffered no more pain, had improved
sleep quality and had regained full range of motion in his neck. He also noted that the tension
headaches had completely resolved. A switch to a more supportive pillow, based on our
recommendations, helped maintain these improvements between sessions.
